WHAT IDENTIFICATION IS NEEDED FOR A LEGAL DNA TEST?
At the time of your appointment, each person will need identification.
Adults (18 years or older) must provide photo identification such as a driver's license, state-issued ID, passport, or military ID.
Minors (under 18 years old) must have identification, but this identification does not have to include a photo. Examples of identification for minors include a birth certificate, certificate of live birth, social security card, health insurance card, or an immunization record. There may be other identification options if the aforementioned are not available. Please contact us for more information.
